I will participate in rehearsing and performing with the TOWNE SINGERS, INC. at my own risk. This includes, without limitations, my participation in any rehearsal, instruction or performance activity, including travel to and from any such activity. I agree that I am voluntarily participating in such activities and in using the facilities and premises leased, rented, owned, managed, maintained, operated or controlled by the TOWNE SINGERS, INC. and/or any of their members or lessors, and I assure the risk of all injury, illness, damage or loss to myself or my personal property that might result from such participation and/or use. This includes the loss or theft of any personal property; it also includes any injury I might suffer as a result of a slip and fall while on premises owned, controlled, managed, operated, maintained, rented or leased by the TOWNE SINGERS, INC. or any of its current, former or future officers and/or members.
I am aware that the TOWNE SINGERS, INC. is a non-profit corporation, without significant assets, and I agree to maintain any insurance that is appropriate to any activity in which I may participate in connection with the TOWNE SINGERS, INC. I agree on behalf of myself (and my personal representatives, administrators, heirs, executors, agents and assigns) to release and discharge the TOWNE SINGERS, INC. (and its members, officers, employees, agents, representatives, successors and assigns) from any and all claims or causes of action, known or unknown, arising out of the negligence of such persons and/or entities.
I acknowledge that I have carefully read this WAIVER AND RELEASE and that I fully understand it is a release of liability. I am waiving any right I may have to bring a legal action or to assert a claim against the TOWNE SINGERS, INC. for its negligence and/or for any action of failure to act by the TOWNE SINGERS, INC. This agreement represents the sole and complete understanding between the parties and it may not be modified except in writing.
